Telemedicina solo por audio y ajuste por riesgo: por qué esos diagnósticos no cuentanAudio-only telehealth and risk adjustment: why those diagnoses don't count

Billing and risk adjustment ask two different questions. Billing asks, "Can this visit be paid?" Risk adjustment asks, "Can this diagnosis be used to measure how sick the patient is?" For Medicare Advantage, an audio-only visit can get a yes to the first and a no to the second.
The rule
CMS accepts diagnoses from telehealth visits for Medicare Advantage risk adjustment when the visit uses real-time, interactive audio and video. Audio-only encounters (reported with modifier 93) do not meet that standard, even when the visit itself is billable (CMS memo, May 4, 2022).
Why it matters
If a chronic condition such as diabetes with complications or heart failure is documented only in phone visits during the year, it may not be captured for that patient's risk score. In a RADV audit, diagnoses submitted from audio-only visits are a finding.
Document the modality every time
- State in the note how the visit happened: audio and video or audio only.
- Record the patient's location and consent.
- Code the visit with the place of service and modifiers your payer requires for telehealth (for example POS 02 or 10, and modifier 95 or 93, or FQ for rural health clinics and FQHCs). Payer rules vary, so check each contract.
- Diagnoses still need MEAT: what the provider monitored, evaluated, assessed or treated.
Build it into your workflow
- Add a required "visit modality" field to your telehealth template.
- Flag audio-only visits so they are kept out of risk adjustment submissions.
- Schedule a face-to-face or video visit during the year for patients whose chronic conditions were only addressed by phone.
