ERA vs. EOB: dos documentos, una reclamación y dónde se pierde su dineroERA vs. EOB: two documents, one claim, and where your money gets lost
The short answer: an ERA (Electronic Remittance Advice) is the payer's payment explanation sent to the provider, built on the HIPAA X12 835 standard. An EOB (Explanation of Benefits) is the plain-language statement sent to the patient.
Same claim, same numbers, different audience and different job.
What is an ERA?
An ERA is the electronic version of a paper remittance advice. Once a payer adjudicates claims, it sends the ERA to the billing provider, usually through a clearinghouse, often bundling many patients into one remit tied to one payment.
Your billing system reads it to post payments, adjustments and patient balances. It carries everything your team needs to reconcile the account:
- Payer and payee (billing NPI and TIN)
- Each patient, claim number and dates of service
- Billed, allowed and paid amounts per line
- Adjustments with group codes, CARCs and RARCs explaining why
- Provider-level adjustments (recoupments, offsets, interest)
- Payment method and the trace number that links it to the deposit

How to read this remit
- Maria Garcia: paid $106.40. The $96.60 in CO-45 is a contractual write-off. The $25.00 copay (PR-3) is the only amount she owes.
- David Lee: allowed but paid $0 because the full $92.10 went to his deductible (PR-1). That is not a denial. Bill the patient.
- Anita Patel: denied CO-16 with RARC M76, a diagnosis problem. It's a fixable error: correct the claim and resubmit; don't write it off.
- Bottom line: the payer kept $20.00 to recover an old overpayment. If your team posts only claim totals, your books show $106.40 and the bank shows $86.40.
The 835 file behind it
The screen above is a translation. The real ERA is an X12 835 file your clearinghouse receives. You don't need to read it daily, but knowing the key segments helps when posted numbers don't match.

- CLP = claim-level payment. SVC = each service line.
- CAS = adjustments: group code (CO, PR, OA, PI) + CARC + amount.
- PLB = provider-level adjustments not tied to one claim. It's the segment most often missed in manual posting.
- TRN = the reconciliation key. CAQH CORE operating rules call for the EFT and ERA to arrive within 3 business days of each other.
What is an EOB?
An EOB is the payer's explanation to the member. It is not a bill. It shows what was billed for each visit, the plan's discount, what the plan paid and what the patient may owe the provider.
Most commercial and Medicare Advantage plans send EOBs by mail or through the member portal. Original Medicare patients get a Medicare Summary Notice (MSN) instead, which does the same job.

Why the patient's EOB matters to you too: when a patient calls to dispute a balance, the EOB in their hand should match your patient statement exactly. If it doesn't, a CO adjustment was likely posted as patient responsibility, and that's a compliance problem, not just a customer-service one.
The adjustment codes that matter
Every adjustment on an ERA has three parts. Read them in this order:
- Group code: who owns it. CO (contractual, provider writes off), PR (patient responsibility), OA (other adjustment), PI (payer-initiated reduction).
- CARC: why. Claim Adjustment Reason Code, e.g. 45 (fee schedule), 97 (bundled), 16 (missing info), 50 (not medically necessary).
- RARC: the detail. Remittance Advice Remark Code, e.g. M76 or N290. It often tells you exactly what to fix.
X12 maintains the CARC and RARC lists and updates them three times a year. Use the official lists at x12.org/codes, not a stale cheat sheet.
Six posting mistakes that drain A/R
These are the patterns we find most often when auditing a practice's payment posting:
Cancelar el CO-45 sin mirar el contratoWriting off CO-45 without checking the contract
Payers underpay contracted rates more often than practices realize.
Solución:Fix: cargue sus tarifas contratadas y marque todo permitido que quede por debajo.load your contracted rates and flag any allowed amount below them.
Offsets sit at the bottom of the remit. Skip them and your deposits never reconcile.
Solución:Fix: postee cada PLB a una cuenta de recobros y lígelo a la reclamación original.post each PLB to a recoupment account and tie it to the original claim.
Tratar denegaciones corregibles como finalesTreating fixable denials as final
CO-16, CO-4 y muchas denegaciones de registro solo necesitan una reclamación corregida.CO-16, CO-4 and many registration denials just need a corrected claim.
Solución:Fix: envíe cada CARC a una cola de trabajo con fecha límite para volver a radicar.route by CARC to a work queue with a resubmission deadline.
Cobrarle al paciente montos COBilling patients for CO amounts
Moving a contractual adjustment to patient balance is balance billing and can breach your payer contract.
Solución:Fix: solo los códigos de grupo PR van a responsabilidad del paciente.only PR group codes post to patient responsibility.
EFT sin ERA (o al revés)EFT without an ERA (or the reverse)
Unmatched deposits sit in suspense while claims age.
Manual posting is slow and error-prone, and paper remits arrive late.
Solución:Fix: inscriba cada aseguradora principal para ERA y EFT con su clearinghouse.enroll every major payer for ERA and EFT through your clearinghouse.
